How to Download the Enhanced Vaccination QR Code

As of January 4th, Ontarians are required to download and save the enhanced QR code vaccine certificate to access several businesses and settings. What this means for the Athletic Centre is that upon arrival, you will be required to show:

  1. Your UofG daily COVID screening checkmark
  2. QR Code enhanced vaccination certificate along with valid ID (examples listed below)

 If you don’t have the QR code yet, you can download it from the provincial website here: https://covid19.ontariohealth.ca/. Once you’ve read and accepted the Terms of use, you will be brought to a page where you’ll be asked to enter your personal information, including your health card number and the 9-character code found on the back of the card. Click continue and you’ll be prompted to download your QR code.

For easier access when you arrive at the gym, you may want to consider adding your vaccination certificate to your Apple Wallet, if you are an Apple user. Make sure you have the iOS 15.1 operating system or later- to update your software go to Settings--> General--> Software Update.

  1. Open your iPhone camera app
  2. Point the camera at your QR code vaccine certificate, then tap and hold on the code.
  3. The Health app will open once the code is recognized and will give you the option to add the vaccination record to your Apple Wallet and Health app.
  4. Now when you want to access your proof of vaccination, double click the lock button on your iPod or iPhone and your vaccination card will appear showing your name, vaccine, date of dose and the scannable QR code.

You can also save a digital copy of your receipt to your device, save a screenshot of the code to your device, or print a paper copy.

Examples of Valid ID:

  • birth certificate
  • driver's license
  • government (Ontario or other) issued identification card
  • passport
  • citizenship card
  • permanent resident (PR) card
  • Indian Status Card or Indigenous Membership Card

 

If you received one or more of your doses outside of Ontario

If you received one or more doses of the COVID-19 covid 19vaccine out of province, contact your local public health unit to register your information and receive proper documentation. They will verify and record your vaccination records in COVaxON. If you do not have an Ontario health card, they can provide you with a unique pass code called a COVID ID.

Once recorded, you will be able to obtain an enhanced vaccine certificate through the provincial portal at COVID-19 vaccination provincial portal or by calling the Provincial Vaccine Contact Centre (PVCC) at 1-833-943-3900. The contact center agent can email you a secure URL so you can access your vaccine certificate.
